Inguinal hernia

What is an inguinal hernia?

An inguinal hernia is a defect of the abdominal wall in the groin, which can be innate and/or acquired. It can result if the groin cannot withstand a load or stress. Under stress the supporting tissue cracks or breaks.

How do I recognize an inguinal hernia?

The abdominal wall in the groin thins off: it creates a natural weakness. The weak point can no longer withstand the internal pressure. The groin starts to break. In most instances one can see a bulge in the groin, but be careful, sometimes hernia may be invisible (occult hernia) from outside and you may demonstrate it only by ultrasound device.

For hernia surgery, there are 3 potential methods:

  • Open – suture method
  • Open – mesh method
  • Laparoscopic

We choose the best method with regard to the individual circumstances and results. We developed a special suture mesh method which combines the advantages of the suture and mesh methods avoid too much foreign material and gives best results without complications and pain.

It has been recognized in meta-analysis of clinical studies in inguinal hernia treatment that the risk for severe complications is increased in laparoscopic hernia surgery. One should discuss the risks and the outcome with regard to these findings. Is it really necessary for groin hernia repair to enter the abdomen? What is the true advantage? What about bowel adhesions caused by intraabdominal operations?
Can these techniques be used outside the hospital and why not? What is the implication for anesthesia?

Umbilical hernia

What is an umbilical hernia?

The human abdominal wall is composed of many layers. In case of an umbilical hernia, the innermost layer, the peritoneum, bulges through a gap formed in the fascia.
Can be triggered by:

  • Weakening of the abdominal wall
  • Overstretching
  • Increase in abdominal pressure

How can I recognize an umbilical hernia?

The umbilical hernia is often seen even from the outside by a bulge in the navel. You can often feel the gap with your fingers. In any case we recommend a sonographic examination to exclude further ventral hernias and to have a clear picture of the extent of the damage in the abdominal wall.

What treatments are there for the umbilical hernia?

In principle, any umbilical hernia in adult life should be surgically treated as an elective procedure. The level of risk is, compared to the risks of an emergency surgery, small.

Other fractions (hernia):

Incisional hernia: The incisional hernia occurs in the area of the cut (incision) from a previous surgery of the abdomen.
Recurrent hernia: A second hernia that occurs after an inguinal hernia surgery, umbilical hernia surgery or incisional hernia surgery.
Sports hernia: A defect in the abdominal wall – without producing any of the commonly described bulging with a hernia sac.
Traumatic hernia: An abdominal hernia caused by an accident.
Ventral hernias: Spigelian hernia, epigastric hernia

Which kind of vein diseases are there?

There are varicose veins of varying degrees (saphenous veins, spider veins and reticular veins) or chronic venous insufficiency.

How do venous diseases become noticeable?

In the initial stage, there are no noticeable indications. In advanced stages, there is swelling, edema, skin lesions and itching,heavy legs, pains, ulcers, thrombosis ….

What are the treatment options?

Firstly, it often helps to eliminate the cause of the disorders (obesity, sedentary lifestyle). Treatment can be conservative with compression stockings and drug or surgical/interventional procedures.
Further treatment may be a type of modern operation avoiding the blue leg (hematoma) and pain in most instances, treatment with sclerosing agents, laser or rediofrequency ablation. Notice, there is no treatment available without side effects! The choice of treatment depens largely on the personal wishes of the patient and the type and changes of the veins.

Therapy:

Most of the diseases in the anal area can be treated conservatively without pain. Otherwise operations can be chosen, but only if no other option is availabe. In most instances hemorrhoids can and should be treated conservatively.
